Would this specific underlay (2.5mm) provide sufficient padding for wood floors overtop of a concrete slab at ground level?

Yes, as long as the wood floor is a floating floor and not a staple or nail down application .

The R-value for our AcoustiCORK Quiet Comfort Underlayment is at minimum 3.125 per inch of thickness.

Can this be used under self adhesive carpet tiles.

We would not typically recommend this since there is a wavy texture to the cork underlay. We have not tested to see if the carpet tile would telegraph the wave.
Is this product good when used on the upper floors of your home on wood sub-floors.
