What is the IIC rating for this product over a 6" concrete subfloor?
The IIC rating for the AcoustiCORK RCLT 5mm Sheet is 51 with glued wood over it and 50 with ceramic tile over it.
I see that the IIC is 51, but what is the STC rating for your 5mm?
The acoustical ratings for AcoustiCORK RCLT 5mm Sheet are as follows.

Can I put this in a concrete basement and then put one of your artificial turfs on top of it?

Yes, you can install the AcoustiCORK RCLT500 5mm Rubber/Cork Boxed Sheet over concrete and place turf over the top. This underlay can be installed over any hard flat surface like concrete, plywood, asphalt, OSB, etc., and can be installed under most flooring types.
